## Episode64: Kinesiology of Sacroiliac joint#9
Iliosacral motion##
บทความที่แล้วผมได้พูดถึงการเคลื่อนไหวภายในเชิงกรานว่าเราแบ่งการเคลื่อนไหวหลักๆได้เป็น3ส่วนคือsacroiliac motion, iliosacral motion, symphysis pubis motionและได้พูดถึงการเคลื่อนไหวsacroiliac motionกันไปแล้ว ในบทความนี้ผมจะพูดถึง2ส่วนที่เหลือคือiliosacral motion, symphysis pubis motionกันครับ
Iliosacral motionคือการเคลื่อนไหวของinnominate boneเมื่อเทียบกับsacrum โดยที่bilateral movementของinnominate bone(anterior/posterior rotation) จะเกิดขึ้นก็ต่อเมื่อมีการflexionหรือextensionของtrunk ในขณะที่unilateral movement(การเคลื่อนไหวเกิดขึ้นแค่ด้านเดียว) จะเกิดขึ้นเมื่อเกิดการflexionหรือextensionของhip jointและlower limbเช่น การเดินเป็นต้น คล้ายๆกับการเกิดsacroiliac motionเลยครับ
สำหรับanterior innominate motion จะเกิดตอนที่hip extension เมื่อinnominate bone rotateไปทางด้านหน้า ที่articular surface(L-shaped)จะglide inferiorlyตามแนวshort arm และglide posteriorlyตามแนวของlong arm การglidingจะคล้ายๆกับการเกิดnutationของsacroiliac joint
ส่วนposterior innominate motionก็จะเกิดตรงกันข้ามกันคือ จะเกิดตอนที่hip flexion และเมื่อinnominate bone rotateไปทางด้านหลังที่articular surface(L-shaped)จะglide superiorlyตามแนวshort arm และglide anteriorlyตามแนวของlong armครับ
การเคลื่อนไหวตำแหน่งสุดท้ายคือsymphysis pubis motion คือการเคลื่อนไหวที่ตำแหน่งของpubic symphysis ซึ่งเป็นจุดเชื่อมระหว่างpelvicทั้ง2ข้าง และระหว่างที่เราเดิน จุดนี้ก็จะกลายเป็นจุดหมุนระหว่างpelvicทั้ง2ข้างด้วย

การเคลื่อนไหวที่ส่วนนี้จะเกิดได้ไม่มากนัก เพราะจะถูกlimitจาก superiorและinferior pubic ligamentครับ
กรณีที่ตำแหน่งนี้มีปัญหาเราจะเรียกว่า “Symphysis pubis dysfunction(SPD)” โดยเราจะclassifiedปัญหาตามpositionที่ข้อต่อนี้ติดอยู่ เช่นsuperior symphysis pubis/inferior symphysis pubisเป็นต้น และการเกิดSPDนี้ก็จะทำให้เกิดปัญหาของsacroiliac jointและiliosacral jointตามมาได้อีกด้วย

SPD จะพบได้บ่อยในผู่หญิงที่ตั้งครรภ์หรือหลังคลอด เพราะว่าligamentภายในเชิงกรานจะหย่อนตัวลง pelvisจะได้ขยายตัวได้มากขึ้นสำหรับการคลอดนั่นเอง
จากบทความที่แล้วที่ผมพูดถึงsacroiliac motionว่าsacrumสามารถเกิดการเคลื่อนไหว nutation/counter-nutation และในบทความนี้ที่ผมพูดถึงiliosacral motion ว่าinnominate bone สามารถเกิดanterior innominate/posterior innominateได้ ทั้งหมดนี้เป็นการพูดถึงindividual motionของแต่ละข้อต่อ ในบทความต่อไปผมจะพูดถึง ความสัมพันธ์ของการเคลื่อนไหวทั้ง2ส่วน รวมถึงความสัมพันธ์กับการเคลื่อนไหวของlumbar spineนะครับ
