Intrinsic muscles of the hand ##
กล้ามเนื้อมัดเล็กๆในมือ หรือที่เราเรียกว่า intrinsic muscles of the hand เป็นกลุ่มกล้ามเนื้อที่มีทั้งจุดเกาะต้นและจุดเกาะปลายอยู่ในมือ มีความสำคัญอย่างมากต่อการทำงานที่ต้องการความละเอียด แม่นยำของมือและนิ้วมือ ในบทความนี้ผมจะพูดถึงกล้ามเนื้อกลุ่มนี้กันครับ
กล้ามเนื้อ intrinsic ในมือสามารถแบ่งออกเป็น 3 กลุ่มใหญ่ๆ ตามตำแหน่งที่อยู่คือ กล้ามเนื้อที่อยู่ด้านนอก(thenar muscles), กล้ามเนื้อที่อยู่ด้านใน(hypothenar muscles) และกล้ามเนื้อที่อยู่ตรงกลางมือ(central compartment muscles)
กลุ่มแรก "Thenar muscles" เป็นกลุ่มกล้ามเนื้อที่อยู่บริเวณฐานนิ้วโป้ง ประกอบด้วยกล้ามเนื้อ 3 มัดคือ opponens pollicis, abductor pollicis brevis และ flexor pollicis brevis
กล้ามเนื้อ opponens pollicis จะเกาะจาก flexor retinaculum และ tubercle of trapezium ไปเกาะที่ lateral border ของ first metacarpal ทำหน้าที่ในการ opposition ของนิ้วโป้ง
กล้ามเนื้อ abductor pollicis brevis จะเกาะจาก flexor retinaculum และ tubercle of trapezium/scaphoid ไปเกาะที่ radial side ของ proximal phalanx ของนิ้วโป้ง ทำหน้าที่ abduction นิ้วโป้งในแนวตั้งฉากกับฝ่ามือ
กล้ามเนื้อ flexor pollicis brevis มี 2 heads คือ superficial head เกาะจาก flexor retinaculum กับ trapezium และ deep head เกาะจาก trapezoid และ capitate ไปเกาะที่ radial sesamoid และ base ของ proximal phalanx ทำหน้าที่ flexion ที่ MCP joint ของนิ้วโป้ง
สำหรับกล้ามเนื้อ adductor pollicis จะอยู่ในกลุ่ม central compartment muscles มี 2 heads คือ oblique head เกาะจาก capitate และ base ของ second และ third metacarpals ส่วน transverse head เกาะจาก palmar surface ของ third metacarpal ทั้งสองส่วนไปเกาะที่ ulnar sesamoid และ base ของ proximal phalanx ทำหน้าที่ adduction นิ้วโป้งเข้าหาฝ่ามือ
กลุ่มที่สอง "Hypothenar muscles" เป็นกลุ่มกล้ามเนื้อที่อยู่บริเวณฐานนิ้วก้อย ประกอบด้วยกล้ามเนื้อ 3 มัดคือ opponens digiti minimi, abductor digiti minimi และ flexor digiti minimi brevis
กล้ามเนื้อ opponens digiti minimi เกาะจาก hook of hamate และ flexor retinaculum ไปเกาะที่ medial border ของ fifth metacarpal ทำหน้าที่ opposition ของนิ้วก้อย
กล้ามเนื้อ abductor digiti minimi เกาะจาก pisiform และ tendon ของ flexor carpi ulnaris ไปเกาะที่ ulnar side ของ proximal phalanx ของนิ้วก้อย ทำหน้าที่ abduction นิ้วก้อยออกจากแนวกลางมือ
กล้ามเนื้อ flexor digiti minimi brevis เกาะจาก hook of hamate และ flexor retinaculum ไปเกาะที่ base ของ proximal phalanx ของนิ้วก้อย ทำหน้าที่ flexion ที่ MCP joint ของนิ้วก้อย
กลุ่มสุดท้าย "Central compartment muscles" ประกอบด้วยกล้ามเนื้อ lumbricals 4 มัด, dorsal interossei 4 มัด และ palmar interossei 3 มัด
Lumbricals เป็นกล้ามเนื้อที่พิเศษตรงที่เกาะต้นจาก tendon ของ flexor digitorum profundus ไปเกาะที่ extensor expansion ทางด้านข้างของนิ้วชี้ถึงนิ้วก้อย ทำหน้าที่ flexion ที่ MCP joint และ extension ที่ IP joints
Dorsal interossei มี 4 มัด เกาะจากด้านข้างของ metacarpals ทั้ง 2 ด้าน ไปเกาะที่ proximal phalanx และ extensor expansion ทำหน้าที่ abduction นิ้วออกจากแนวกลาง(นิ้วกลาง) และช่วย flexion ที่ MCP joint กับ extension ที่ IP joints
Palmar interossei มี 3 มัด เกาะจากด้านเดียวของ metacarpals ไปเกาะที่ proximal phalanx และ extensor expansion ทำหน้าที่ adduction นิ้วเข้าหาแนวกลาง และช่วย flexion ที่ MCP joint กับ extension ที่ IP joints เช่นกัน
กล้ามเนื้อกลุ่มนี้ทั้งหมดได้รับเส้นประสาทมาจาก median nerve(thenar muscles ยกเว้น deep head ของ flexor pollicis brevis) และ ulnar nerve(ที่เหลือทั้งหมด รวมถึง adductor pollicis) ซึ่งถ้ามีการบาดเจ็บของเส้นประสาทเหล่านี้ก็จะส่งผลต่อการทำงานที่ละเอียดของมือได้ครับ
