กลไกการออกฤทธิ์
Azelastine ยับยั้ง H-1 receptor agent จึงมีประสิทธิภาพในการรักษา allergic rhinitis ส่วน Fluticasone propionate ยับยั้ง proinflammatory mediators เช่น histamine, leukotrienes, cytokines, และ
eicosanoids เป็นต้น และ cell types เช่น mast cells, eosinophils, lymphocytes, macrophages, และ neutrophils เป็นต้น
Efficacy Evidence
จากการศึกษาประสิทธิภาพของยา Dymista แบบ Meta-analysis 3 การศึกษาจำนวนผู้ป่วย 3398 คน และ 4 การศึกษาจำนวนผู้ป่วย 610 คน เป็นการศึกษาแบบ randomized โดยเกณฑ์คัดเข้าผู้ป่วยที่มีอายุมากกว่า 12 ปี ที่มีอาการปานกลางถึงรุนแรงของโรค allergic rhinitis ที่รับการรักษาด้วย placebo หรือ Dymista หรือ intranasal เดี่ยวร่วมกับ azelastine or fluticasone propionate ระยะเวลาในการรักษา 14 วัน Primary end-point คือ อาการคัดจมูก คัน น้ำมูกไหลและจาม.
สรุปผลการศึกษา
ทั้ง 4 การศึกษาให้ผลเหมือนกันในด้านการลดอาการคัดจมูก คัน น้ำมูกไหลและจาม โดยพบว่าผู้ป่วยที่ใช้ยา Dymista มีการตอบสนองมากกว่า 49 %
อาการข้างเคียง
• เลือดกำเดาไหล
• ระคายเคืองบริเวรที่สูดพ่น
• ปวดศีรษะ
สรุป
Dymista น่าจะเป็นตัวเลือกที่มีประโยชน์สำหรับผู้ที่เป็นโรคจมูกอักเสบจากโรคภูมิแพ้ ช่วยบรรเทาอาการได้เร็วกว่าการใช้ Fluticasone propionate หรือ azelastine แบบเดี่ยว
